Media Release Form
CONCENT
I, the undersigned, hereby grant Pure Pixel Media, LLC, (the “Presenter”), its affiliates, agents, and assigns, the irrevocable and unrestricted right to record, photograph, and/or film me, my likeness, voice, and appearance (collectively, the “Materials”).
I consent to the use of the Materials by the Presenter for any purpose, including but not limited to advertising, marketing, social media, promotional materials, publications, and any other media, whether now known or hereafter developed, in perpetuity and worldwide, without additional compensation, notice, or further consent.
I waive any right to inspect or approve the finished product or the use of the Materials. I release, discharge, and agree to hold harmless the Presenter, its affiliates, employees, and agents from any and all claims, demands, or causes of action that I may have, including those arising from the use of the Materials, whether for libel, defamation, invasion of privacy, or any other claim.
I represent that I am at least 18 years of age and have the full legal capacity to execute this release, or, if under 18, that my parent or legal guardian has signed below. Entering my name into this electronic form constitutes an electronic signature and my consent to the use of electronic signatures.
I understand this release is binding and cannot be revoked.
